About CHARM-COPD

To improve the health outcomes, the experience, and the population health of older adults transitioning to home after a hospital admission for Chronic Obstructive Pulmonary Disease (COPD) exacerbation, our plan is to refine, scale and evaluate a Connected, at-Home, Accessible Remote Monitoring in COPD (CHARM-COPD) Platform.
